Please have the following with you in order to register:
- Most recent W-2 forms or four paycheck stubs
- Child's Social Security card
- Original State Birth Certificate
- Bill or lease to verify address in the city of Chesapeake
- If receiving WIC, food stamps, or TANF, please bring your WIC number, food stamp case number and your social worker's name.
