Required Paperwork | Day Camp
REQUIRED PAPERWORK
Because YMCA summer day camp is a state-licensed child care program, certain paperwork is required for your child to attend. After reserving your child's spot, you will receive a welcome email from our partner Playerspace, where you can:
- Create your parent portal account (this will be a separate login from your YMCA Account)
- Upload your child's immunization record, physical, and ID verification (birth certificate or report card)
- Finish your child's camp registration by completing the Child Health History & Emergency Contacts in Playerspace
- Submit medication authorization forms and allergy forms as needed
Through our online parent portal, you'll also receive updates and timely communication from your camp staff this summer!
